Must be seeking medical treatment at a hospital that is a member of the Malaysia Healthcare Travel Council (MHTC).
Must obtain an appointment letter from an MHTC member facility prior to application.
Passport must be valid for at least 6 months from the date of travel.
Must be outside of Malaysia at the time of application.
Must provide proof of sufficient funds for the duration of the stay.
Must have a confirmed return flight ticket.
