PUBLISHED REQUIREMENTS
Must be a Ukrainian citizen, a person granted international protection in Ukraine before Feb 24, 2022, or a foreigner with permanent residence in Ukraine who cannot safely return home.
Family members of the above categories (if they resided in Ukraine before Feb 24, 2022) are also qualifying.
Must apply in person at a Foreign Police Department or designated center.

Valid passport or other identification document.
Birth certificate for children.
Proof of accommodation (lease agreement, affidavit from property owner, or confirmation from an accommodation facility).
